Download it and printed off here:

So get it filled out and ready to be mailed on your brithday (and mail it before going to bed) and its all good!  (Just make sure you don't sign it and put in a date before your brithday wait till your brithday for the dates  )
Also note, your one of the younger guys here, but I am a young guy too! Only alittle over 2 years over your age!
By the way if your going to shorten my name I perfer Big D.

Well I think it was Bushy that started calling me Big D, and I was a bigger "D" then I am now, I'm 6ft 2in and I was 240 pounds.
Well you can try printing it off and if there is no ink (I know the feeling I let it run dry all the time  ) you'll know.

Well Daniel, I for one am not insulted or offended.....I reckon your Dad has given you some pretty bloody good advice. But then again, I'm only a young fella too.
I was born and raised in the "wilds" of Sydney....largest city in Australia...but I am in reality a country boy at heart. The attutudes of city folk are usually...GO GO GO GO...don't slow down...make more money...to hell with the "other guy"...what's in it for me...none of them, or very few of them, seem to know how to relax. Me I like to kick back in the scrub, do a bit of hunting, then sit around the fire enjoying a beer, the company of me mates and telling lies to one another.

Weinbender, yeah we've got a couple of you young fellas (ahem, us young fellas  ) Don't worry we're all here for the same thing basically and I for one am more than impressed with the respect that young people conduct themselves with in this forum, we must draw the right kind of people I reckon

Eric! Scotch Accent? I usually have that when I'm starting to slur my words after about the ninth or tenth shot. Scottish accent in Delta? I suppose there's a few, you should talk to someone from Newfoundland or Cape Breton, you'd swear you were in Ireland or Scotland.
