I use it on my shotties if not broken open... pump gets used a fair bit but in saying that it is usually slung on shoulder if going walkies...it gets left semi open when in the boat or on vehicle, on centrefire hardly ever used,only if lining up on animal from prone and it moves so waiting for it to turn etc.

After rewatching the clip I am not sure the safety failed. This type of safety is designed to prevent both trigger pull and hammer fall. It prevented trigger pull until it was placed in the fire position. We can't tell regarding hammer fall. These were slam fires. The firing pin is held back by a spring so inertial pin movement is unlikely unless the spring is missing, broke or the wrong length. Had that happen in a FAL. The problem might be the firing pin was jammed and extruding from bolt. More information required.

Not sure if they are designed to fail, but people put too much trust in them.
Designed by man and therefore falible, perhaps.
Does winchester do a firing test on their firearms before they send them out?
